LOCATION: Center is relocating to the city of GRAND TERRACE in San Bernardino County. It will be located at 22400 Barton Road. Must be able to commute to this location once the new facility site is licensed by the State of California Department of Public Health.

POPULATION SERVED: Mostly ambulatory adults with mental illness residing in licensed residential care facilities.

WORK HOURS: 30 hours (M-F 8:30am-3pm OR 9am-3:30pm)

OVERALL OBJECTIVE: Develop the nursing component in the individual plan of care for each participant capable of benefiting from the programs and services of the Center.

QUALIFICATIONS:

• Registered Nurse licensed by the California Board of Registered Nursing.
• Must have medical examination and Tuberculosis (TB) clearance.
• CPR and First Aid training certificates.
• Other requirements of the State of California (for example, must be fully vaccinated for COVID-19 and have 1 COVID-19 booster).

Responsibilities:

• Be directly responsible to the Program Director and serve as a member of the Multidisciplinary Team (MDT) to assess the nursing needs of the participant.
• Be on duty during hours of operation to be the primary point of observation and monitoring of each participant's health status to assist participants in maintaining optimum health.

DUTIES:

• Conduct initial and ongoing assessment of each participant to determine needs
• Create a care plan and supervise the provision of those services and/or provide the services.
• Document all skilled nursing care provided on the day service is given and include quarterly signed and dated progress notes in the participant's record.
• Provide health related training to staff.
• Train and supervise Program Aides in personal care duties.
• Monitor, administer and record prescribed medications.
• Provide liaison to the participant's personal physician.
• Supervise the implementation of the maintenance therapy program by Program Aides after being trained by the Physical and Occupational Therapists.
