Job description

Provide case management in-home services to frail elderly by conducting ongoing assessment of client's long-term care needs. Coordinate a comprehensive care plan and implement in-home services to ensure client's needs are met so they can stay at home rather than go to a nursing facility.

Requires a licensed social worker or registered nurse with at least one year experience in home care for the elderly and/or disabled adults, home health care or other relevant experience required. Ability to legally drive a car or make transportation arrangements; ability to access private home/locations that may not be readily accessible; basic medical terminology a plus; good documentation skills required; ability to work with client's family, doctor, and service providers.
