MCS Health is a locally owned healthcare agency dedicated to providing long term care for individuals inside and outside of their home. We are currently seeking a dynamic and energetic Physical Therapist (PT) for client Case Management. Our office is located right in downtown Springfield, MA!

Responsibilities and Skills of Physical Therapist (PT)
- Complete all applicable clinical assessments.
- Develop and review members' plan of care
- Monitor health status of all members and document accordingly
- Conduct on-site PT visits
Requirements and Qualifications
- Physical Therapist must be fully licensed in the Commonwealth of Massachusetts
- Ability to independently organize work and meet deadlines.
- Bi-Lingual Spanish/English Preferred
- Minimum of 2 years of experience in multiple therapy environments.
- Adept at working with the geriatric, orthopedic and general medical populations.
- Strong interpersonal abilities and manual therapy skills
- Independent transportation.
Responsibilities:
- Evaluate and provide ongoing PT treatment to effectively correct patients’ movement difficulties and advance their functional abilities.
- Arrange and maintain schedules with patients.
- Travel to patients’ homes with necessary (provided) equipment operating on an accurate schedule.
- Communicate effectively with patients and practice manager.
